My Health. My Wellness. Toolkit

What does it mean to be active in your health and wellness?

Self-determination is an important part of being active in your health and wellness. To be self-determined means to set your own life goals and make your own decisions.

Whether you decide to go to your doctor’s visits alone, or with a support person, it is important that you have a say in the health decisions being made.

Self-Determination and
Your Health

A large part of self-determination is setting your own goals. When it comes to your health, it is important to set health goals for yourself and to talk with your doctor to figure out the best way to reach your goals. Everybody’s health goals will look different. Some examples of health goals could be to get better sleep during the week or to drink more water throughout the day.
